Financial Aid Forms 2010-2011

These forms are to be used for Fall 2010, Spring 2011 and Summer 2011. 

 

Supplemental Information Form

All students must complete the college's Financial Aid Supplemental Information Form. This form is returned directly to the LPC Financial Aid Office. Information from the Supplemental Information form takes precedence over information from the FAFSA. This form will be sent to the student with a 'Missing Documentation Letter'

Ability to Benefit (ATB) Test

This test is required for students who do not have a high school diploma or GED in order to qualify for financial aid. The ‘ATB’ test is a test of basic math skills and reading comprehension. The federal government requires that all students who do not have a high school diploma or G.E.D. must demonstrate passing scores on this test before they are eligible to receive any federal financial aid. This test is also required to receive a Cal Grant. Only Fee Waivers are exempt from this requirement.

PLEASE NOTE: Effective July 1, 2009, you are exempt from the ATB requirement if you have successfully completed 6 units of academic coursework at Las Positas. Please contact the Financial Aid Office if you would like us to review your transcripts to determine if you meet this qualification.

Academic Competitiveness Grant (ACG) Validation Form

Recent high school graduates who qualify for a Pell grant may be eligible for an ACG Grant for $750 for their first year of college and $1300 for their second year. The student must have completed an ‘eligible rigorous course of study’ in high school, which includes the California A-G requirements, or The California Golden State Seal Merit Diploma, or aset of courses similar to the State Scholars Initiative.. The ACG Validation form must be completed by the student's high school, who will verify if the student met the eligible rigorous course requirements. The ACG Validation Form may not be completed prior to high school graduation.

Verification Forms

Some students are randomly selected by the federal government for mandatory verification of income. The information in the student's SAR will indicate the student has been selected for verification. Verified students are required to provide the financial aid office with documentation of all sources of income for the previous tax year and must submit a Federal verification form.

Low Income Certification Forms

Students whose income (or whose parent's income if dependent) is below reasonable subsistence level must file this form upon request in order to explain how they were able to meet their basic living expenses on such low reported income.

The process of determining eligibility for financial aid uses standard formulas and standard student budgets. The Financial Aid Office recognizes that some students have unusual, special circumstances that are not considered in the standard process of determining eligibility. Students with special circumstances are encouraged to contact the Financial Aid Office or download a form for whichever special circumstance is most appropriate.

Dependency Override Form

It may be possible to have your dependency status officially changed by the Financial Aid Officer by filing a Request of a Change in Dependency Status petition with the financial aid office. There must be extenuating circumstance (such as a family situation involving abuse or other adverse circumstances) involved which are documented by a professional familiar with the situation in order to be considered.

Enrollment Fee Waiver Form (BOGW)

The Board of Governors Enrollment Fee Waiver Program was created to provide access for California residents to Community Colleges so that the cost of enrollment would not be a barrier. You may be required to submit documentation of income with your form.
