Certificate of Liability Insurance

Proof of Liabiltiy Insurance for Renting/Using off-campus facilities in the community for College Use.

To provide the requestor with a Certificate of Liability Insurance from Las Positas College, please fill out the Liability Certificate of Coverage Request form and contact the Executive Assistant to the Vice Chancellor of Business Services.

The District Business Services Office will need to know:

1. The date and time of the event;
2. Who is sponsoring it and the participants;
3. The address of the certificate holder;
4. If they need to be additionally insured. If so, they will need a contract for use of facilities.
