If you require a paper or alternate format of any financial aid document, please contact the Financial Aid Office at (925) 424-1580 or visit us during office hours.
| In order to complete your financial aid file each year, you will need to supply all forms and documents that are requested (found on CLASS-Web, written tracking letters, and/or staff requests). Each must be completed, signed and dated, with appropriate documentation attached. No financial aid will be awarded or disbursed until your file is complete. |
Please bring all of your requested financial aid follow-up documents to Las Positas College Financial Aid Office during our office hours. We may request additional documents after our initial review. After the review has been completed, if you are eligible for financial aid, you will receive an award letter which will inform you of types and amounts of financial aid for which you are eligible. Please view your financial aid file and award status on CLASS-Web Financial Aid, rather than calling or visiting our office, to allow our staff more time to process and award student's files.

2) Drop off forms in our after-hours drop box outside our door
3) Scan as a .pdf file, or save as .pdf, and attach to an email sent from your zonemail account

Agency Certification & Untaxed Income |
Submit to your case worker or the Social Security Office for completion and return to our office. If using this form to establish eligibility for BOG A, you must submit it with a completed BOG application.

| Board of Governor's Fee Waiver Application |
The Board of Governors Enrollment Fee Waiver Program was created to provide access for California residents to Community Colleges so that the cost of enrollment would not be a barrier.
Please Note:
You are required to submit a copy of your 2012 income tax form, as proof of income, with your fee waiver application form.

| Child Support Paid Verification |
On your Free Application for Federal Student Aid (FAFSA) or your Verification Worksheet, you indicated that you, your spouse (if married) or your parent(s) (if dependent), paid child support during the year 2012. As part of the verification process, you must confirm the amount of child support paid, by whom the child support was paid, and to whom it was paid.

There are two separate petitions with different instructions, depending on your academic goal:
If your academic goal is TRANSFER:
1. Ensure all prior college transcripts for which you wish to receive credit are received by the college before submitting.
2. Complete first page and top of second page of Petition form.
3. Complete attached Student Educational Plan Preparation Request for Financial Aid Petition and submit to the Counseling Desk, Building 700 with your petition.
4. Attend SEP workshop at least 5 working days after you submit your form, where you can pick up your completed petition.

| High School Completion Verification |
On your Free Application for Federal Student Aid (FAFSA) you indicated your high school completion status. You have been selected for verification of this information. If you will complete (or have already completed) high school or its equivalent (i.e. GED) before the 2013-2014 school year, you must confirm and provide any documentation requested.
| Identity Verification |
The student must appear in person at the Financial Aid Office to verify his or her identity by presenting a valid government issued photo identification (ID), such as, but not limited to, a driver’s license, other state issued ID, or passport. The institution must maintain a copy of the student’s photo ID that is annotated with the date it was received and the name of the financial aid specialist authorized to collect the student’s ID.

| LPC Grant Hold |
Request to place a Pell or Cal Grant on hold. Effective July 1, 2012, students are limited to 600% (6 Full-Time equivalent years) of Pell Grant eligibility in a LIFETIME. Pell payments received are added together for all colleges attended anywhere in the United States for all years of aid.

| Reinstatement Petition Form |
For students who have been placed on suspension due to poor progress. You must attend an SAP Workshop prior to filing this petition. You must attach a Student Education Plan (SEP) to your petition. Complete attached Student Educational Plan Preparation Request for Financial Aid Petition and submit to the Counseling Desk, Building 700. Attend SEP workshop at least 5 days after you submit your form, where you can pick up your Student Education Plan.

| Request for Consideration of a Change in Dependency Status |
In unusual hardship cases, the Financial Aid Office may be able to assist a student who is technically dependent if the student can make a compelling case showing that it is unhealthy or impossible to provide the parental information.

Submit form to Admissions and Records Office, Building 700, for a refund of your enrollment fees. This form is used when you have a credit balance on your account due to receiving a fee waiver after paying for your classes.

Circumstances include but are not limited to loss of a job, loss of taxable or untaxable income, unusual medical expenses, separation or divorce, death in the family, etc.
